If you are a new client interested in scheduling an appointment for therapy, please choose the counseling option and follow any further instructions.
PRIVATE INSURANCE
If your private insurance provider is:
-BCBS
-United healthcare
-Webtpa
-Aetna
-Cigna
-Tricare
Or any other private insurance provider.
Please complete your information and MFT will verify your benefits. Based on that verification, we will email you an estimate. However, the insurance information we have on file is not always up-to-date, so the best we can do is give you an estimate of your treatment costs. We strongly recommend contacting your insurance company before contacting MFT in order to supply us with the most accurate information.
COMMERCIAL INSURANCE
If you have a commercial insurance provider, please select Intake Request and select your preferred appointment time. This is a requested appointment time, but it is not confirmed until your therapist contacts you to either confirm or reschedule the appointment. If you do not hear from someone within 24 hours of your appointment time, please call or email the office.
SOONERCARE
If you are insured by Soonercare, please select the Soonercare Intake option. Select “reserve without paying.” You will not be required to store your card information.
Note: MFT will do our best to meet your preferences, but understand that appointment times are limited.
NEUROPSYCHOLOGICAL TESTING
For neuropsychological testing, please select that option. We do not currently bill private insurance for this service so you will be required to pay for your service upfront, and then you can submit the claim to your insurance and possibly get reimbursed your out-of-network amounts according to your policy. The first half of this fee is required when you book the appointment. You will need to pay the second half in order to receive your results, which can take up to 30 days.
GIFT CERTIFICATE
Please call or email the office to schedule an appointment if you have a gift certificate.
Nutrition ReferralS
A referral from your primary doctor must be completed before services are performed. Please send Nutrition Referral Form or fax it back to us 405-300-0646.
